Discount Program

Sliding Scale

We are glad to be able to help our neighbors, and we treat all patients alike, regardless of their class, race, age, gender or handicapped status.

We have a Sliding Scale program that can be used at all health center locations as well as a pharmacy program that is valid at Pill Box locations in Franklin and Harman. The Sliding Scale program reduces the cost of care to our patients and is based on Federal poverty levels. Patients with insurance can also benefit from this program as discounts can be applied to deductibles and co-payments. If you qualify for this program, you will have a set fee for doctor visits and could receive assistance with prescriptions through the pharmacy.

To receive a discount on the cost of care, you must meet with one of our Outreach Assistance Workers and complete the Sliding Scale Application

On the Application form, you will need to list all family members, adults & children, living in your household. You will also need to provide proof of income of those living in the household. Listed below are acceptable documents.

  • Pay stubs showing last four weeks of income

  • Unemployment Compensation showing last four weeks of income.

  • Workers Compensation showing last four weeks of income.

  • Social Security Income (SSI)

  • Disability or SSA Benefits Letter

  • Documentation from a local, state or federal agency

  • Last year’s income tax return including W-2 or 1099 (only if Self-Employed)

Your household size and combined income will determine your rate based on our sliding scale table.

This discount is good for up to one year. We will send out a letter stating when you are due for review, if you have any major changes before then, please contact our office.

Referral Programs

We work with a number of organizations to help meet the needs of our patients and other individuals.

PCC can assist individuals with food insecurity. We have a referral form that can be completed in office and taken to the local C.A.N. (Christians Assistance Network) establishment to obtain their services. This is based on household size and gross monthly income:

PCC does not offer dental services on site.  However, we have a local dental facility that participates with our sliding scale program.  When you sign up for our sliding scale, if you would like your information faxed to that dentistry office, we would be happy to refer you.
