If you or a loved one is unable to travel to our health center, whether due to transportation issues, surgeries, or general health, the Home Visit Program team would love to become part of your medical team. Please contact the care manager at (304) 358-2355 to set up a visit today.

Services and Providers

The Home Visit Program treated their first patient on February 4, 2015. The team is comprised of a nurse practitioner and a care manager who travel to people’s homes and conduct medical visits.

  • Immunizations
  • Flu Shots

The Home Visit team conduct the same services you would see in an in-clinic visit: monitor vital signs, perform quality management measures, perform blood draws, conduct diabetic foot exams, and do head-to-toe assessments. They see patients with chronic illness as well as people who are acutely sick. They conduct appointments Monday through Friday from 8am to 4:30pm.

The team services the greater Pendleton Community Care service area – which consists of Pendleton, Randolph, and Grant County in West Virginia and Highland County in Virginia.

To qualify for the Home Visit program, a patient must have a chronic disease process (for example Diabetes, Hypertension, or Congestive Heart Failure). However, these are not the only diseases that would warrant you a visit. Another qualifying measure is that you must be “home bound” or have limited access to your medical doctor. This means that you can’t drive, don’t have a car, have limitations visually, and have a chronic disease that would cause you undue harm to make the trip to the clinic. Again, these are only examples and do not exclude a patient if you don’t have these limitations. The other qualifying parameter is that at this time any and all patients in the Home Visit patient base MUST be patients of Pendleton Community Care and already be established with one of the providers there. If you would like to take advantage of the Home Visit Program at Pendleton Community Care and do not already see a doctor, please call and set up an initial visit with a PCC provider.
